How to Get Started

Getting started with IBHS is easy. Just call us at (909) 708-8158 to register and schedule your first appointment.

What to Bring to Your First Visit

To make sure we have everything we need to provide you with the best care, please bring the necessary information to your first appointment. You can save time by filling out the registration form before you arrive. You’ll need a:

  • Valid photo ID, such as a driver’s license or passport.
  • If the address on your ID isn’t up to date, please bring a recent utility bill or bank statement that shows your current address.
  • Bring your Social Security card, if you have one
  • If you’re insured, please bring your
  • Insurance ID cards, as well as your
  • Medicare or Medi-Cal cards (if applicable)

Forms You’ll Need

Before your appointment, it’s helpful to complete a few more forms so we can better understand your health needs. Forms are available in both English and Spanish.

Adults aged 18 and over, will need to complete the:

  • Consent to Treat Form
  • Health Information Exchange (HIE) Consent Form
  • Adult Health History Form

If you’re bringing in a minor, you’ll need to fill out the:

  • Consent to Treat Form
  • Health Information Exchange (HIE) Consent Form
  • Pediatric Health History Form

If you’re employed, bring a recent paycheck stub or last year’s tax return for all working family members.

If you’re unemployed, bring proof of unemployment and any other sources of income.

Our discount program is available to all patients, whether or not you have insurance.

Support at Our Clinics

If you have any questions or need help, our team is here for you. Visit one of our Certified Eligibility Counselors (CEC) or Patient Services Representatives at the clinic’s registration desk for assistance. We can help with discount programs, federal and state assistance programs, accepted insurance plans, and photo ID requirements. We also offer registration help for minors and provide bilingual support in English and Spanish. If you need a sign language or foreign language interpreter, just let us know.
